Talk to your GP team about reviewing your advance directives and getting your advance care plan loaded onto their system.
Please keep an original of your advance care plan and take it with you to health appointments or ED. Remember to keep your advance care plan up-to-date and share any changes with your GP team.

Make an appointment with your GP or practice nurse to help you with the medical aspects of your advance care plan.
Your medical centre can put your signed and dated plan onto your medical centre file and either you or your medical centre can send a scanned copy plan to acp@ccdhb.org.nz to have it added to your hospital file.
This allows hospital doctors across the region to access and respond to your wishes in an emergency. If you change your plan in the future, share the new version with your medical centre and acp@ccdhb.org.nz
