Presented in Easy Read format, it was co-designed in Aotearoa New Zealand with researchers with learning (intellectual) disabilities, disability service managers, whānau and health professionals.
Guided by Dr Nic McKenzie, the co-researchers developed the template to help people be involved in the advance care planning process, which – in its usual format – can be hard for people with learning disabilities.
The additional Guidebook for supporters helps supporters adapt the planning process to suit each person’s needs.
